Once you have been given an appointment, you will be sent an email containing your login and password. You will then go onto your patient portal and fill out all of your registration information. This will allow you to avoid filling out the forms in the waiting room.

If you cannot access the patient portal, please download, print, fill out and bring all three completed forms to your testing visit:
